It involved hacking up my old wiper arm, a lot of work with a Dremel, epoxy, JB Weld metal filler, etc. Was worth the effort though.

Wiper Arm - 86532AG082 - $28 USD
Cover for Nut - 86538AG010 - $3 USD
Wiper Blade (Your old blade will not work) - 86542AG080 - $15 USD
If you decide to do this, it's also the perfect time to replace any other wiper components that need replacing. I had a crack in the plastic nozzle spayer assembly (86636FA130 - $15 USD) and the outer rubber seal was weathered and cracking (86536FA030 - $2 USD).
