Question

In the group stages, if three teams finish with "equal" records then the winner and runner up are decided by the results against the other "equal" teams. What happens if there is no difference there either?

If every team in the England group were to beat them 1-0 but all other matches in this group were 0-0 draws, who would not qualify? (Apart from England)

Alternatively who would qualify from a group if all the matches in that group were draws with the same score, eg 1-1?

I know the second scenario is not very likely but it is possible. There must be something in the tournament rules to cover the eventuality.
