Web design Help, "submit form"
 

Hands up, very ametuer at web design.
I have done a couple of basic websites using Frontpage (I know I know).
I am reasonably happy with them now, apart from I want to add a submit form type page.
You know the sort of thing I guess, like this form HERE

I have googled and found some info on how to go about it, but haven't done any sort of script before, so I'm not getting very far at all.
I found out how to do the e-mail form but that looks a bit crappy even for my site:D

Any help or links to idiots guides on how to achieve this sort of thing would be appreciated,
