I have a Ver 4 Type R with a fully forged 2.5 to drop in but like yourself i have the ver5-6 head's and phase 2 inlet manifold.

Do you have the measurements of that billet alloy block you had machined for the IACV??
And how you plug the Old Iacv hole above the throttle body ?
