You can turn on a passcode feature that requires any new logins to input a code that it sends to your phone.

This - you can set up trusted browsers, then anyone trying to log in from an 'untrusted' browser will be asked for a random passcode which FB will generate for you. Change your password as a first measure though, any site is easy to hack if you have a password that's easy to guess

Depends on where your password reset requests go.

If that's an email account she has access and she's changed the ID verification to a mobile phone number thats in her possesion then obviously she can get back in whenever she likes.
