Was looking for a more accessable option than the backpack for body plus 100-400 lens and found these:

I tried one that went on a belt on holiday once and it ended up in the bin, anywhere it was OK to access, ti was uncomfy to wear, anywhere it was comfy to wear, it was difficult to access and made sitting anf getting in and out of cars a bit awckward.
I went with backpack for long distance transport and a courier style bag for walking around. Not obviously a camera bag and can be swung round out of the way or round the front. Crumpler do loads but I'm using a John Lewis bag at the moment that suits me. Not suitable for a trek but just right for walking around and at trackside.

I'm thinking more for those occasions when I am out and about in less than great weather with the 100-400 on the 1d2n and need some protective case. Belt wear really isn't an option with that combo - would be shoulder strap only.

My 100-400mm L lives in a ThinkTank DH30. Fits like a glove, either with or without hood extended. Actually, I pack mine with the hood reversed, and use the extendable base section as a handy home for an Extender.
Great bag, lots of neat features, but you will need the DH50 with the bigger top section to accommodate the extra depth of your camera. Love mine :)

PS Also a perfect fit for a 70-200mm f/2.8 L.
